Please Print, complete and return all four pages. These are not "online" forms but must be completed on paper and received in the NIACC Records Office before your PSEO registration is complete

1. To cancel a registration [PDF] prior to the start of the term use this form. 

2. To withdraw from all courses [PDF] after the term has started use this form. 

3. To make a change in a schedule [PDF] use this form.
