A student must first register with Disability Services before accommodations should be requested. It’s also necessary to meet with the Director of Disability Services, either in person or remotely using Zoom, the first time accommodations are being requested or when changes to accommodations are being requested.

This form may only be used once accommodations have already been established for at least one semester and only when the accommodations being requested remain the same as in the previous semester.
